The Generic Medical Document feature allows providers to create and send official notes or referrals directly from the Sesame provider portal.
This tool can be used for:
Sick or Absence Notes
Letters of Medical Necessity (LOMN)
Imaging Orders
Custom Documents
How to Create a Medical Document
Open the patient’s chart.
Click the paper icon to open the Medical Document tool.
Choose the appropriate document type from the menu:
Sick / Absence Note – for school or work excuses.
Letter of Medical Necessity – for insurance, equipment, or prior-authorization needs.
Imaging Order – for radiology or diagnostic imaging referrals.
No Template – creates a blank, Sesame letterhead document that you can customize for any purpose.
Enter the required information or write your own notes.
Click Submit to finalize the document.
After submission:
The document will automatically appear in the patient’s chart.
The patient will receive a copy in their Sesame portal and can download it anytime.
Using the Document for Lab Orders
If a specific lab test isn’t available through Sesame’s integrated lab ordering system, you can create a generic lab order using this same tool.
Choose No Template, and write the lab order manually.
The patient will receive the document in their portal and email.
They can take it to any lab of their choice.
Because this order is outside Sesame’s system, the patient must pay cash or use insurance directly at the lab.
Tip:
All documents created through the portal automatically appear in the patient’s chart. No need to email separate PDFs—everything is logged and accessible in one place.
