Provider Requirements
To participate in the Costco Partnership Program:
Set your primary care visit pricing to $24 (provider price), which displays to Costco members as $29 (patient price).
Set your online therapy pricing to $74 (provider price), which displays to members as $79 (patient price).
Any primary care service you offer at the eligible pricing—including telehealth visits, refill visits, UTI visits, etc.—will be automatically included and listed on the Costco + Sesame portal.
You do not need to offer all services at this price; only those that meet the eligible pricing will appear for Costco members.
