As a participating clinician, you lead the evaluation and optimization process at a primary-care level, guiding patients from initial workup through referral when needed, while building longitudinal relationships and predictable recurring revenue.

About Sesame Subscriptions

Sesame’s subscription programs give providers an opportunity to offer ongoing, personalized care while generating predictable monthly recurring revenue. Patients who subscribe tend to stay longer, stay healthier, and deliver higher lifetime value for your practice.
How subscriptions work:
When a patient enrolls in one of your Sesame subscription programs, they begin with an initial video visit to establish care, review their goals, and set a treatment plan.
After that, you can manage care in the way that best fits your workflow and clinical judgment:
Offer inclusive follow-up visits for care related to the subscription.
Conduct asynchronous check-ins through Sesame Messaging for ongoing management or refills.
Prescribe one month at a time or several months at once, when clinically appropriate.
Providers are paid monthly, when the patient’s subscription renews.
Subscriptions allow you to deliver consistent, proactive care while giving patients a convenient way to stay connected to you and their treatment plan.

This program is a structured fertility pathway designed at a primary care level of scope.
The Sesame clinician functions as the clinical entry point and longitudinal anchor. Your role is not that of a reproductive endocrinologist, but rather a primary-care-level fertility architect who ensures patients are appropriately evaluated, optimized, and referred when needed.
Every patient starts with Sesame.
Every patient completes a comprehensive intake and diagnostic workup with their Sesame clinician.
From there, the Sesame clinician determines next steps:
Continue management within Sesame (optimization pathway)
Delay specialty referral while addressing modifiable factors
Refer to IVI RMA when clinically appropriate
IVI RMA provides high-acuity procedural and specialty care. Sesame provides continuity, preparation, and coordination.

Comprehensive reproductive intake and timeline
Ordering and interpreting baseline labs and imaging

PCOS (Metformin, lifestyle modification)
Thyroid dysfunction
Prolactin abnormalities
Metabolic factors impacting fertility
Coordinating evaluation of male factor as part of the overall fertility workup
Ordering genetic testing (optional; may expedite IVF/IUI coordination if completed early)
Generating referral to IVI RMA when clinically appropriate
Providing longitudinal support before, during, and after specialty care

What the First Two Visits Look Like
Visit 1: Comprehensive Intake & Workup
You should document:
Duration of unprotected intercourse
Cycle regularity and ovulation tracking
Prior pregnancies or losses
Relevant surgical or STI history
Male partner history
You will:
Order baseline fertility lab workup (refer to Clinical Guidance for protocol)
Order transvaginal ultrasound via Green Imaging
Initiate semen analysis coordination when appropriate
Begin addressing modifiable risk factors if indicated
The focus of Visit 1 is full diagnostic activation.
Visit 2: Results Review & Pathway Decision
You will:
Review labs and imaging

Clarify whether the patient is best suited for:
Continued optimization within Sesame
A defined period of conservative management
Referral to IVI RMA
If referring, generate the referral through the Sesame system. (see clinical guidance for correct template)
The referral expands the care team — it does not replace the Sesame clinician relationship.
Documentation & SLA Requirements
Document Gravida/Para (G/P) status in every note
Document LMP in every note
Review and release labs within 48 hours
Respond to patient messages within 24 hours
