As a participating clinician, you lead the evaluation and optimization process at a primary-care level, guiding patients from initial workup through referral when needed, while building longitudinal relationships and predictable recurring revenue.

About Sesame Subscriptions
Build stronger patient relationships and sustainable revenue.
Sesame's subscription programs give providers an opportunity to offer ongoing, personalized care while generating predictable monthly recurring revenue. Patients who subscribe tend to stay longer, stay healthier, and deliver higher lifetime value for your practice.
How subscriptions work:
When a patient enrolls in one of your Sesame subscription programs, they begin with an initial video visit to establish care, review their goals, and set a treatment plan.
After that, you can manage care in the way that best fits your workflow and clinical judgment:
- Offer inclusive follow-up visits for care related to the subscription.
- Conduct asynchronous check-ins through Sesame Messaging for ongoing management or refills.
- Prescribe one month at a time or several months at once, when clinically appropriate.
Providers are paid monthly, when the patient's subscription renews.
Subscriptions allow you to deliver consistent, proactive care while giving patients a convenient way to stay connected to you and their treatment plan.
Fertility Subscription Overview
Patient price:
- $119/month (Standard)
- $99/month (Costco Members)
This subscription covers virtual care and coordination only. Labs, imaging, medications, and fertility procedures (e.g., IVF/IUI) are not included in the subscription price.
Provider price:
- Starting at $50/month (you set your price - we'll adjust the patient price commensurately)
What's included for patients:
- Comprehensive fertility intake
- Diagnostic coordination (labs and imaging)
- Personalized care plan for underlying conditions and optimization guidance
- Referral to IVI RMA when clinically appropriate
- Ongoing messaging and follow-up support throughout their journey
- Access to discounted pricing on labs, imaging, and IVI RMA procedures (available only to active Fertility program subscribers)
Core Care Model
This program is a structured fertility pathway designed at a primary care level of scope.
The Sesame clinician functions as the clinical entry point and longitudinal anchor. Your role is not that of a reproductive endocrinologist, but rather a primary-care-level guide who ensures patients are appropriately evaluated, optimized, and referred when needed.
Every patient starts with Sesame.
Every patient completes a comprehensive intake and baseline diagnostic workup (strongly encouraged; required prior to IVI RMA referral)
From there, the Sesame clinician determines next steps:
- Continue management within Sesame (optimization pathway)
- Delay specialty referral while addressing modifiable factors
- Refer to IVI RMA when clinically appropriate
IVI RMA provides high-acuity procedural and specialty care. Sesame provides continuity, preparation, and coordination.
Scope of Practice
In Scope: Sesame Clinician
- Comprehensive reproductive intake and timeline
- Ordering and interpreting baseline labs and imaging
- Managing optimization of underlying conditions:
- PCOS (Metformin, lifestyle modification)
- Thyroid dysfunction
- Prolactin abnormalities
- Metabolic factors impacting fertility
- Prescribing ovulation induction medication when clinically appropriate (see clinical guidance for instructions on accessing discounts for Costco members)
- Generating referral, inclusive of all baseline workup requirements (see clinical guidance), to IVI RMA when clinically appropriate
- Providing longitudinal support before, during, and after specialty care

What the First Few Visits Look Like
Visit 1: Intake, Context, and Goal Setting
The priority of this visit is listening and understanding, only starting on diagnostics or referral if that's a patient's best next step.
You will:
- Review intake, documenting fertility history and medical context
- Define patient context and goals: Where is the patient in their fertility journey?
- Early-stage (not yet evaluated; unsure where to start)
- Mid-journey (some prior workup; exploring options like ovulation induction)
- Advanced (recent diagnostics completed; seeking IVF/IUI or egg freezing referral)
- Gather any prior labs, imaging, or specialist notes, identifying gaps in workup
- Order any labs or imaging as needed (e.g., transvaginal ultrasound, FSH, AMH)
- Begin addressing modifiable risk factors if indicated
The goal of Visit 1 is alignment and direction, not instant referral.
Visit 2: Results Review & Treatment Planning
The priority of this visit is thoughtful interpretation and shared decision-making.
You will:
- Review labs and imaging, explaining findings in the context of the patient's goals and timeline
- Clarify whether the patient is best suited for:
- Continued optimization within Sesame
- A defined period of conservative management
- Escalation toward procedural care (IUI/IVF/egg freezing)
Important: Not all patients require a specialist referral. Take time to align on a plan before moving forward.
Visit 3 and ongoing: Optimization, Follow-up, and Referral Readiness
The next priority is progression: either continuing optimization or preparing for referral.
You will:

Assess response to treatment plan (if optimizing)
- Lifestyle changes
- Medications (e.g., Metformin, thyroid management)
- Review any outstanding or newly completed diagnostics
- Refine or adjust care plan based on response and results

If appropriate, begin preparing referral package:
- Ensure all required labs, imaging studies, and tests are complete
- Confirm results have been reviewed with patient
Referral may be initiated at any time in the patient's journey with Sesame, but only when clinically appropriate and fully prepared.
Communication Expectations
Providers are expected to respond to patient messages within 24 hours. Clear communication and timely follow-up are part of this program's core value.
A note on patient communication: Many patients entering this program are particularly sensitive, feel a sense of urgency, and are full of questions about treatment options and pricing. It's crucial to slow down and listen, making sure patients feel supported and confident moving forward with next steps.
