The Success by Sesame (SBS) program offers providers an opportunity to support patients in achieving their weight loss goals through comprehensive treatment options. This program ensures high-quality care by combining medical expertise, treatment plans, and seamless communication.

About Sesame Subscriptions

Build stronger patient relationships and sustainable revenue.

Sesame’s subscription programs give providers an opportunity to offer ongoing, personalized care while generating predictable monthly recurring revenue. Patients who subscribe tend to stay longer, stay healthier, and deliver higher lifetime value for your practice.

How subscriptions work:
When a patient enrolls in one of your Sesame subscription programs, they begin with an initial video visit to establish care, review their goals, and set a treatment plan.
After that, you can manage care in the way that best fits your workflow and clinical judgment:

  • Offer inclusive follow-up visits for care related to the subscription.

  • Conduct asynchronous check-ins through Sesame Messaging for ongoing management or refills.

  • Prescribe one month at a time or several months at once, when clinically appropriate.

Providers are paid monthly, when the patient’s subscription renews.

Subscriptions allow you to deliver consistent, proactive care while giving patients a convenient way to stay connected to you and their treatment plan.

Success by Sesame Overview

  • Patient price: $99/month
  • Suggested provider price: $36/month
  • Patient intake: Patients complete a short form with medical history and current prescriptions.
  • Initial visit: Review intake during a video consult and create a personalized care plan.
  • Ongoing care: Manage prescription refills, adjustments, and labs when needed. Patients who have the Sesame app will be prompted to complete weekly check-ins, their responses accessible to you on their chart.
  • Unlimited patient-provider messaging: Patients can reach out to their provider at any time for medication-related support. For clinical concerns outside the scope of their Success by Sesame subscription, patients are encouraged to book a separate one-time appointment.

Provider responsibilities include:

  • Completing the initial consult with new Success by Sesame patients.
  • Ordering labs when appropriate (cost of labs associated with weight loss treatment/diagnosis are included in the program, excluding patients in NY, NJ, and RI)
  • Managing renewals, adjustments & refills. Reminders will appear in your task list. Advise patients on dosing instructions where needed.
  • Completing prior authorizations as needed.
  • Responding to patient messages within 24 hours and maintaining appropriate follow-up cadence.

Follow-Up Appointments

Follow-ups included in the subscription are not publicly bookable. Patients must use your direct follow-up booking link. This should be in your go-live email for this program. Please email doctors@sesamecare.com if you cannot locate this link.

  1. Navigate to Sesame ePrescribe.
  2. Select: Zepbound Subcutaneous Solution Auto-Injector
    • Doses available: 2.5mg/0.5ml, 5mg/0.5ml, 7.5mg/0.5ml, 10mg/0.5ml, 12.5mg/0.5ml, 15mg/0.5ml
  3. Add to prescription:
    • Instructions: "Inject [DOSE]mg subcutaneously once weekly"
    • ICD-10 code
  4. Choose a pharmacy under Local Pick-Up:

Zepbound Vials

  1. Navigate to Sesame ePrescribe.
  2. Select: Zepbound Subcutaneous Solution (vial — not “auto-injector”).
  3. Add to prescription:
    • Instructions: "Inject [DOSE]mg subcutaneously once weekly"
    • ICD-10 code
  4. Send to: LillyDirect Cash Pay for Zepbound Vial
  5. Select Send Order.
    • LillyDirect will contact the patient directly to arrange payment & shipping.

💲 Zepbound Vial Pricing:

  • 2.5mg: $299 for a 4-week supply ($74.75 per vial)
  • 5mg: $399 for a 4-week supply ($99.75 per vial)

Zepbound KwikPen

  1. Navigate to Sesame ePrescribe.
  2. Select: Zepbound Subcutaneous Solution (KwikPen).
  3. Add to prescription:
    • Instructions: "Inject [DOSE]mg subcutaneously once weekly"
    • ICD-10 code
  4. Send to: LillyDirect Cash Pay for Zepbound KwikPen
  5. Select Send Order.
    • LillyDirect will contact the patient directly to arrange payment and shipping.

Each KwikPen contains 4 fixed doses (one-month supply).

💲 Zepbound KwikPen pricing is same as vial pricing.

Wegovy Pen

  1. Navigate to Sesame ePrescribe.
  2. Select: Wegovy Subcutaneous Solution Pen-Injector (correct dose).
    • Example starting dose: 0.25 mg/0.5 mL
    • Other doses: 0.5 mg/0.5 mL, 1 mg/0.5 mL, 1.7 mg/0.75 mL, 2.4 mg/0.75 mL
  3. Add to prescription:
    • Instructions: "Inject [DOSE]mg subcutaneously once weekly"
    • ICD-10 code
    • Dispense quantity (e.g., 2 mL for a 28-day supply at 0.25 mg)
    • Days supply: 28 days
    • Check Dispense as Written (DAW)
    • Notes: "Please dispense brand name Wegovy®, not to be confused with other semaglutide-containing products."
  4. Select a pharmacy under Local Pick-Up:
    • If patient using insurance, select patient preferred pharmacy
    • If cash-pay, select NovoCare® Pharmacy (search zip code 32809)
      • Standard doses: $349 (4-week supply)
      • Intro pricing as of November 17, 2025: $199/month - first 2 months for new patients
  5. Select Send Order.

Wegovy Pill - see prescribing guide

  1. Navigate to Sesame ePrescribe.

  2. Select: Wegovy Oral Tablet (correct dose).

    • Starting dose: 1.5 mg tablet

    • Step-up doses: 4 mg, 9 mg

    • Maintenance dose: 25 mg

  3. Add to prescription:

    • Instructions: "Take [DOSE] mg by mouth once daily upon waking, on an empty stomach, with a sip of water (4 oz max). Wait at least 30 minutes before eating, drinking, or taking other oral medications."

    • ICD-10 code

    • Dispense quantity: 30 tablets (30-day supply)

    • Days supply: 30 days

    • Check Dispense as Written (DAW)

    • Notes: "Please dispense brand name Wegovy® oral tablet. Do not substitute with injectable semaglutide or other semaglutide-containing products."

  4. Select a pharmacy under Local Pick-Up:

    • Mail Order (RECOMMENDED): Select NovoCare® Pharmacy

      • Cash-pay pricing through NovoCare® Pharmacy:

NOTE: Patients must present a Novo Nordisk savings card at pickup to access advertised cash pricing.
Instruct the patient to text SAVE to 83757 before going to the pharmacy (fastest option).

Compounded GLP-1s

Please refer to our September 2025 clinical guidance on compounded GLP-1s.

Please note: Compounded GLP-1 access is narrowing nationwide as brand-name supply stabilizes and regulatory scrutiny increases. As branded GLP-1 medications continue to become more accessible and pricing becomes more competitive, many patients will ultimately need to transition to name-brand options. We encourage clinicians to begin planning for this longer-term shift when discussing treatment plans with patients.

  1. Select “Weight Loss Program”

  2. Choose “Compounded GLP-1 medication (cash pay by patient)”

  3. Select a medication from the currently available options:

  5. A template will automatically populate. You may edit as needed before submitting.

    • SIG: Inject [dose] mg subcutaneously once weekly.

    • Quantity / Days’ supply: QS for 28 days at prescribed weekly dose.

    • Only 4-week doses may be prescribed.

      • NOTE: Do not add refills to the prescription - the patient will have to pay for all refills at once.

    • Add syringes and alcohol swabs if needed.

    • Include BMI/diagnosis, prior therapy or access barriers, and a brief statement of medical necessity for compounded therapy.

  6. After submission, Sesame emails the patient a payment link. Once payment is completed, the designated pharmacy (HealthWarehouse or Epiq) ships the medication directly to the patient.

 

Liraglutide

Sesame does not currently offer an integrated mail-order delivery option for liraglutide. However, clinicians can still prescribe liraglutide for local pharmacy pickup if it is appropriate for the patient.
